TransCanalicular Endoscopic Diode Laser Dacriocystorhinostomy; Surgical Techniques and Clinical Outcomes

Background: Chronic dacryocystitis is an inflammatory condition of the lacrimal sac that develops as a result of nasolacrimal duct obstruction persisting for longer than six months. External dacryocystorhinostomy (DCR) has long been considered the standard treatment for chronic lacrimal obstruction. Transcanalicular dacryocystorhinostomy (DCR) using laser represents one of the latest minimally invasive alternatives in nasolacrimal surgery. Objective: The study aimed to analyzes success rate, procedure time and complications for transcanaliculus DCR with a 980 nm diode laser in a series of 50 successive procedures. Patients and Methods: A prospective study was conducted at Al-Hilla General Teaching Hospital and Al-Salam Private Hospital between March 2023 and April 2026. The study included 50 patients,18 males and 32 females, aged between 8 and 74 years who presented to the ENT department after referral from the ophthalmology department with a diagnosis of chronic dacryocystitis. Results: Most of patients in 41-50 years ,more common in female resulting (male: female) ratio of ( 1:1.7 ). Most Common operated side was left, the overall success rate was 88%. Conclusions: The transcanalicular -DCR with bicanalicular intubation was a minimally invasive, quick procedure yielding results comparable to classic DCR, and better than endoscopic-DCR that can do local anesthesia and for pediatric patient. It enables fast patient recovery, a short procedure time and avoidance of skin scar.
